Details
Any project involving human participation must have the following documents completed and signed by all human participants PRIOR to completing the experiment:

Download an example of the Information Letter

Download a blank Information Letter. Use it to explain your project. Give one to every participant to keep.

Download a blank Informed Consent Form – permission. Every participant must sign one. The Adult Supervisor must store them securely. These must be shredded after the science fair season is over.
Approval of Low Risk Projects
After you have finished your project, and before you arrive at the Regional Science Fair:

Download Form 4.1a Approval of Low Risk Projects.

Fill it in and have it signed.

Follow the instructions at the end of the form.
Approval of Significant Risk Projects.
After you have finished your project, and before you arrive at the Regional Science Fair:

All projects involving vertebrate animals and cephalopods must be approved BEFORE any research starts. Fill in your research plan BEFORE you start your research. Submit it to the Head of the Ethics of your Regional Science Fair.
